What is medicinal leech therapy?

Hirudotherapy, also known as medicinal leech therapy, is an ancient therapeutic method involving the application of medicinal leeches. It has a long history of traditional use, while medicinal leeches are also used today in selected areas of modern medicine.

During treatment, a leech removes a small amount of blood while releasing biologically active substances from its saliva into the local tissues. The best known of these is hirudin, which is recognised for its anticoagulant action.

The therapeutic action therefore combines local blood removal with the local activity of substances contained in leech saliva, particularly their effects on coagulation and local microcirculation.

Why are medicinal leeches used?

Medicinal leech therapy is primarily used for its local effects on congestion and microcirculation. Its action combines the removal of a small amount of blood with the local effects of biologically active substances contained in leech saliva.

Depending on the individual situation, the intended effects may include:

  • reducing local congestion
  • supporting local microcirculation and blood flow
  • influencing certain local inflammatory processes
  • supporting certain localised painful or tense areas
  • assisting in selected cases of tissue congestion or haematoma.

The suitability of leech therapy must always be assessed individually. It does not replace medical diagnosis or conventional medical treatment when these are required.

The indication for Jalaukāvacaraṇam depends on the condition, the patient’s general health and individual Ayurvedic assessment. The available scientific evidence is not equally strong for every traditional use of medicinal leeches.

Osteoarthritis, joints and localised pain

Medicinal leech therapy has been studied particularly for certain types of pain associated with osteoarthritis. It may be considered in selected joint conditions involving local pain, inflammation or congestion.
From an Ayurvedic perspective, assessment also considers the patient’s constitution and the nature of the underlying imbalance rather than focusing exclusively on the painful area.

Circulation and local venous congestion

Medicinal leeches are particularly known for their use in selected situations involving venous congestion and impaired local microcirculation.
An assessment is necessary to understand the cause of the problem and to exclude vascular conditions requiring specific medical investigation or treatment.

Local inflammation

Jalaukāvacaraṇam may be considered in selected situations involving local inflammatory and congestive features.
Within Ayurvedic assessment, characteristics traditionally associated with Pitta and Rakta may also be taken into consideration.

Skin conditions and Ayurvedic dermatology

What happens during a Jalaukāvacaraṇam session?

Assessment

Before treatment, we review the reason for your consultation, general health, medical history, medication, allergies and possible contraindications. When the therapy forms part of Ayurvedic care, your constitution and the nature of the imbalance are also considered.

Application of medicinal leeches

The treatment area and number of leeches are selected according to the indication. Medicinal leeches are placed on the chosen areas and generally detach naturally after feeding.

Bleeding and dressing

After the leeches detach, local bleeding for several hours is expected, partly because of the anticoagulant substances present in leech saliva. An appropriate absorbent dressing is applied.

Aftercare

You receive clear instructions regarding the dressing, hygiene, rest, normal post-treatment reactions and the signs for which you should contact L’essencce.

Before & After Leech Therapy: Preparation, Precautions and Safety

Before your session

The application area must be clean and free of scented products. On the day of your appointment, do not apply to the area:

  • perfume or aftershave
  • body creams or oils
  • essential oils
  • strongly scented skincare products
  • medicated creams or ointments unless specifically advised
  • strongly perfumed soap or shower gel immediately before treatment.

Leeches are sensitive to odours, and products applied to the skin may interfere with their attachment.

Wear loose, comfortable clothing that allows easy access to the treatment area and leaves enough space for the dressing afterward. If leeches are being applied to the foot or ankle, bring open or sufficiently wide footwear that will not compress the dressing.

After your session

Because the treated area may continue to bleed for several hours, plan a quiet and restful day following treatment. You should:

  • drink sufficient water or herbal tea
  • rest after your appointment
  • keep and monitor the dressing according to the instructions provided
  • avoid scratching the bite sites
  • wear clothing that does not put pressure on the treated area.
Avoid gym workouts, weight training, running, strenuous exercise and significant physical exertion after your session. Ideally, do not schedule intensive physical activity on the day of treatment.

What should I tell the practitioner before leech therapy?

It is essential to inform us of all medication, allergies, previous illnesses and previous reactions before treatment, in particular:

  • anticoagulant or antiplatelet medication
  • aspirin or other blood-thinning products
  • bleeding or clotting disorders
  • haemophilia
  • anaemia
  • diabetes or impaired wound healing
  • significant immune suppression
  • important allergies or previous allergic reactions
  • tendency to develop keloid scars
  • any significant ongoing illness or medical treatment.
Never stop prescribed medication on your own in preparation for medicinal leech therapy.

Contraindications: when may leech therapy not be appropriate?

Medicinal leech therapy is not suitable for everyone. An assessment before treatment is therefore essential.

Leech therapy may be contraindicated or require particular caution in situations including:

  • significant bleeding or clotting disorders
  • haemophilia
  • significant anaemia
  • certain anticoagulant or antiplatelet treatments
  • significant immune suppression
  • important wound-healing problems
  • certain serious or progressive medical conditions
  • significant tendency to form keloid scars
  • pregnancy or other situations requiring individual assessment.

Aspirin, blood thinners and other medication

If you take aspirin, anticoagulants, antiplatelet medication or other medication affecting blood clotting, you must inform us before your appointment.

People taking antiplatelet or anticoagulant medication (including aspirin) require individual assessment and should not stop prescribed medication without guidance from the clinician responsible for it.

What are the possible side effects of medicinal leech therapy?

Following treatment, prolonged local bleeding, redness, mild swelling, sensitivity or itching may occur. These local reactions are usually temporary.

Less commonly, infection, delayed wound healing, persistent scarring or a more significant allergic reaction may occur. A large blister, burn-like reaction, increasing swelling, significant pain, spreading redness, fever or unusual discharge should be reported to L’essencce.

Do not puncture a blister yourself and avoid self-treating an unusual reaction without appropriate advice. Difficulty breathing, swelling of the face or throat, severe dizziness or other signs of a serious allergic reaction require urgent medical attention.

Note: Patients taking antiplatelet or anticoagulant medication require individual assessment before treatment. Do not stop, interrupt, or change any prescribed medication without first consulting the clinician who prescribed it.

Side effects

Frequently and during the days following the treatment, swelling and redness of the treated area is common. In 80% of cases, itching of the bite area occurs. It is important not to scratch as this delays healing and you risk infecting the wounds. Swelling of the lymph nodes in the treated area is also possible and disappears within a week without consequences. Rarely anaphylactic shock occurs.

Delayed healing is sometimes possible, especially in predisposed patients (diabetes). Small whitish star-shaped scars may also remain, or a keloid scar may appear in predisposed individuals.

Frequently Asked Questions About Medicinal Leech Therapy

The initial bite may feel like a small prick or brief burning sensation. Mild sensitivity, redness or itching may develop around the application sites afterward.

Bleeding for several hours is expected after the leeches detach. An absorbent dressing is applied; contact us if the bleeding appears unusually heavy or difficult to control.

Come with clean skin and avoid perfume, creams, body oils, essential oils or strongly scented products on the treatment area. Wear loose, comfortable clothing that leaves sufficient room for the dressing afterward.

Tell us about all medication, allergies, medical conditions and previous reactions, particularly blood thinners, aspirin, bleeding disorders, anaemia, diabetes, immune suppression or wound-healing problems.
