Understanding Heavy Periods

Heavy menstrual bleeding, often referred to as menorrhagia, describes periods whose volume or duration significantly affects quality of life.

In practice, this may involve very frequent pad or tampon changes, repeated night-time leakage, double protection, large clots or unusually long periods.

The amount of blood is not the only important factor. Fatigue, dizziness, pallor, shortness of breath or reduced exercise tolerance may suggest iron deficiency or anaemia and should be medically assessed.

Why Can Periods Become Very Heavy?

There are many possible causes, and heavy bleeding should not automatically be attributed to a “hormonal imbalance”.

Possible causes include uterine fibroids, endometrial polyps, adenomyosis, ovulation disorders, hormonal changes, bleeding disorders and certain medications.

Age and clinical context also matter. A recent change in menstrual flow, bleeding between periods or bleeding after menopause requires appropriate gynaecological evaluation.

Understanding Heavy Periods According to Ayurveda

In Ayurvedic reasoning, excessive menstrual bleeding may be considered particularly in relation to Rakta Dhatu (the blood tissue in Ayurvedic physiology) and Pitta (the principle of transformation and heat) when the presentation has corresponding features.

Apana Vata (the downward-moving function of Vata associated with pelvic and menstrual functions) may also be considered in the assessment of the menstrual cycle.

However, when prolonged bleeding has caused fatigue or weakness, the Ayurvedic approach must also consider general strength, digestion and nutritional status. The goal is therefore not simply to “reduce Pitta”.

The Ayurvedic Approach to Heavy Periods

Assessing the Cycle, Bleeding Pattern & General Health

The consultation explores the duration of menstruation, intensity of flow, frequency of pad or tampon changes, presence of clots, pain and any bleeding between periods.

Fatigue, sleep, digestion, bowel function, diet and current medication are also reviewed. Where available, gynaecological findings and blood tests such as haemoglobin and ferritin can provide important context.

02 Step 02

Individual Ayurvedic Assessment

The Ayurvedic assessment distinguishes Prakriti (individual constitution) and Vikriti (current imbalance) and may consider Pitta, Vata, Apana Vata, Rakta Dhatu and Agni (digestive and metabolic capacity) according to the individual presentation.

Heavy bleeding with sensations of heat does not necessarily require the same strategy as prolonged menstruation accompanied by pallor, weakness, digestive fragility and exhaustion.

03 Step 03

Nutrition, Iron & Recovery

Ahara (nutrition) is adapted to the degree of blood loss, Agni, nutritional status and individual needs.

When iron deficiency or anaemia has been confirmed, the priority is to ensure appropriate nutritional and medical management rather than impose restrictive diets.

Food habits are reviewed to support adequate, digestible and restorative nutrition, especially when heavy periods are causing significant fatigue.

Ayurvedic Herbal Medicine & Therapies

Ayurvedic formulations may be considered after assessing the individual terrain, bleeding pattern, digestion and current treatments.

Particular care is required when the patient is using hormonal treatment, medication affecting blood clotting or iron supplements.

Body therapies are not automatically indicated. Abhyanga (Ayurvedic oil massage), Svedana or Nadi Svedana (general or localised therapeutic sweating) and other heat-based therapies should be selected with caution according to the intensity of bleeding, menstrual phase and overall condition.

Panchakarma: An Indication That Requires Careful Assessment

Panchakarma (a group of classical Ayurvedic therapeutic cleansing procedures) is not an automatic response to menorrhagia.

A patient with active heavy bleeding, marked weakness or anaemia first requires appropriate medical assessment and stabilisation.

Any later consideration of Ayurvedic procedures depends on the cause of bleeding, constitution, Agni, general strength, menstrual pattern and current treatments.

Management Should Be Guided by the Cause of Bleeding

Heavy bleeding caused by a submucosal fibroid, endometrial polyp, ovulation disorder or another condition does not require the same medical or complementary strategy.

Ayurveda does not replace investigation of the cause. It may be integrated into an individualised approach addressing constitution, digestion, nutrition, recovery and selected symptoms alongside appropriate gynaecological care.

Clots can occur when menstrual flow is heavy, but large or newly frequent clots combined with very heavy bleeding should be evaluated to identify the underlying cause.

Very heavy bleeding accompanied by faintness, severe weakness, dizziness, shortness of breath, palpitations or a feeling of impending collapse requires prompt medical assessment rather than complementary care alone.

Yes. Ayurvedic care may complement conventional treatment, but hormonal medication, antifibrinolytic treatment, iron supplementation or other prescribed medication should not be changed without medical advice.
